Ea82 overheating

Hey all,
Have posted about this before but havnt sorted it out.
My car has overheated on several occasions. once when coming back from holidays running at highway speed, it was up close (but not on) the red area for a long way, and then when i was going to the airport the other day, in peak hour traffic, it was pretty hot the whole time.
When i got out of the traffic, and got moving on the freeway at ~ 100km/h it didnt cool down at all, but once I pulled off the highway and went onto normal 60km/h roads it started to cool down. I cant figure it out, as it overheats at highway speeds and at low speeds, but not in between.
When i turn the aircon on, the thermo fan turns on, so the fan is working. I run around with the aircon off at all times, and have never heard the fan kick in otherwise. Clutch fan seems to be working fine. Do i need to replace the thermo?
The other thing i'm worried about is that it's a head gasket, but it doesnt overheat in suburban driving, and it doesnt come up to temperature any faster than any other subaru i've ever owned.
Am going to flush the radiator too see if that does anything.
Can anyone give me a hand/suggestions?
Also, because I HAD to run it at these temps to get people to the airport, is it possible that this could damage my engine? (never in the red, but close)
other issues are a lumpy idle, exhaust bursts (not backfire but like a quick rush of air, and doesnt seem to be blowing steam, oil not milky) that goes with the lumpy idle.
Any help would be good, thanks.
